Alignment with Commission Goals:
Women of Color on the Move’s Sacramento Family Stability Project directly aligns with the Commission’s goal of assisting destitute men, women, and children by addressing both immediate survival needs and long-term stability.
Our program provides:
- Emergency assistance, including food, clothing, hygiene supplies, and transportation, ensuring no individual goes without basic necessities.
- Housing support for families in crisis, through temporary housing assistance and connections to shelters and rental programs.
- Pathways to self-sufficiency, such as case management, job readiness workshops, and referrals to healthcare and workforce services.
By combining direct relief with long-term stability strategies, our program reduces the hardships of poverty and homelessness while empowering participants to move toward safety, health, and independence. This comprehensive approach ensures that Sacramento’s most vulnerable residents are not only cared for in the moment, but also supported in building a stronger and more stable future.

Our program aligns with ALBH Annual Funding Priorities by directly assisting destitute Sacramento residents through food, clothing, and emergency housing support, while connecting them to health, education, and employment services.

Case Manager Responsibilities

Intake & Assessment
- Conduct comprehensive intake interviews to identify immediate needs, including food, housing, and transportation.
- Verify Sacramento residency and determine eligibility based on crisis circumstances or income level.
Service Coordination
- Connect clients to emergency food assistance, clothing, hygiene supplies, and transportation support.
- Facilitate referrals to shelters, rental assistance programs, healthcare providers, mental health services, and workforce development resources.
Individualized Action Plans
- Develop personalized short-term and long-term action plans focused on stability and self-sufficiency.
- Monitor client progress and adjust service plans as needs evolve.
Advocacy & Client Support
- Advocate on behalf of clients with partner agencies, service providers, and landlords.
- Provide emotional support, guidance, and crisis intervention as needed.
Documentation & Reporting
- Maintain accurate and timely case notes, service records, and client documentation.
- Track outcomes and service utilization to support program evaluation and funding compliance.
Follow-Up & Long-Term Stability
- Schedule regular follow-up check-ins to ensure continued progress and engagement.
- Connect clients to ongoing community resources that promote long-term stability and independence.
